GO MD USA LLC is an independent program coordination and monitoring group with offices at 3385 Airways Blvd, Ste 201, Memphis, TN 38116. This Privacy Policy describes how we collect, use, disclose, and protect information collected through gomdcare.org and related services.

Information you provide: Name, phone number, email address, ZIP code, insurance type, preferred call time, and any information you choose to share with our program coordination team.

Automatically collected: IP address, browser type, device identifiers, pages viewed, referring URL, UTM parameters, and timestamps.

From third parties: Insurance verification information from your carrier when authorized by you.

3. How We Use Information

  • To respond to your inquiry and contact you about the Health Link monitoring kit and associated care programs.
  • To verify insurance coverage and determine eligibility for program benefits.
  • To provide and administer monitoring services if you enroll.

By submitting our intake form and checking the consent box, you authorize us and our authorized representatives to contact you by phone (including autodialer and prerecorded voice), text message, and email. You may opt out at any time by:

  • Replying STOP to any text message we send.
  • Calling 833-706-3872 or any number we use to contact you.
  • Replying to any email with "unsubscribe" or clicking the unsubscribe link.
  • Emailing unsubscribe@gomdcare.org.

A single opt-out request applies across all phone numbers, text channels, and email addresses we use to reach you.

We use cookies and similar technologies for site functionality, analytics, and conversion tracking. We do not use third-party advertising pixels that transmit sensitive information from this site.
