Block 1 — prior to eligibility check

Notice Regarding Participation in a CMS Payment Model

The services offered by GO MD USA are provided as part of a new federal payment model test being conducted by the Centers for Medicare & Medicaid Services (CMS), known as the Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model. By proceeding, you acknowledge that you have been informed of the following:

  • Data Sharing. In connection with ACCESS, GO MD USA may share your health information with CMS. CMS may also access or request your health information — including through Health Information Exchanges (HIEs) or other sources — in connection with the program's evaluation. Any such sharing is subject to applicable federal privacy and security protections.
  • Control Group Assignment. You may be randomly assigned to a control group and be ineligible to enroll in ACCESS with any participating health care provider for 12 months, unless otherwise stated by CMS. Assignment to the control group will not otherwise affect your Medicare benefits, rights, or coverage.

Consent to Receive ACCESS Services

The services offered by GO MD USA are provided as part of a new federal payment model test being conducted by the Centers for Medicare & Medicaid Services (CMS), known as the Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model. By enrolling, or aligning, with GO MD USA to receive ACCESS services, you acknowledge the following:

  1. Voluntary Alignment. Your alignment with GO MD USA in the ACCESS model is entirely voluntary. You are not required to align, and your Medicare benefits will not be affected by your decision to align or not align.
  2. Cost-Sharing. Your cost-sharing amount associated with these services is up to $0 per month for the duration of your alignment with GO MD USA.
  3. Right to Disenroll (Unalign). You may end your alignment with GO MD USA — and, if you choose, align with another participating health care provider — at any time beginning 90 days after your initial alignment date.
  4. One Provider Per Clinical Track. You may only be aligned with one participating health care provider per ACCESS clinical track at any given time. However, you may be aligned in multiple different clinical tracks simultaneously — with the same or different health care providers — for each clinical track in which you have a qualifying condition.
  5. Claims Data Sharing. Your Medicare claims data may be shared with GO MD USA, subject to all applicable privacy and security protections, including the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

What is the ACCESS Model?

The Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model is a CMS Innovation Center test of a new payment approach that supports Medicare beneficiaries with chronic conditions through virtual, integrated care alongside your existing providers. GO MD USA is a participating provider offering all four clinical tracks.

Will this affect my Medicare benefits or cost me money?

No — your Medicare benefits, rights, and coverage are not affected. GO MD USA sets your ACCESS cost-sharing at $0 per month, so there is no out-of-pocket cost to you while you remain aligned in the program.

Can I enroll in more than one track?

Yes. The ACCESS model lets you be aligned to multiple clinical tracks at the same time if you have a qualifying condition for each (for example, both heart/kidney issues and behavioral health needs). Select every area that applies on the form and our team confirms eligibility for each during intake.

What if I am assigned to the control group?

If CMS randomly assigns you to the control group, you will be ineligible to enroll in ACCESS with any provider for 12 months. You keep all of your usual Medicare services. This assignment is only for program evaluation and is not based on your health.

How long does the program last? Can I leave early?

You may end your alignment (unalign) at any time beginning 90 days after your initial alignment date, and align with another ACCESS provider if you wish. Your Medicare coverage continues regardless.

What is the phone loaner program?

If you need reliable connectivity for calls, secure messaging, or sending monitoring readings, GO MD USA can provide a phone with voice + data while you are in ACCESS. The phone is for connectivity only — no apps are currently installed, and future apps may be released. If you leave the program we send a prepaid return label, or you may move to a cash-pay plan to keep the phone.

Will my doctor be involved?

Yes. GO MD USA sends care updates to your primary care provider or referring clinician, who may be eligible to bill CMS for co-management. You can continue seeing all of your usual providers.
